Whether it is the start of a new school year or you are new to Haynes, there is a lot to know and a lot of forms to complete. Information provided on this page is intended to get you started. Please keep in mind, forms need to be completed at the start of each school year even if all your information remains the same.
Back to School Forms
All Back to School forms can be found on the Forms and Publications page. Forms include, but are not limited to, health and emergency contact, permanent dismissal, and photographic permission. Students who require administration of medication and/or require an allergy care plan will need to complete various medical forms that can be found on the SPS Nursing page. Parents wishing to volunteer are required to complete a parent volunteer form, CORI form and volunteer contract.
Directories, Notepads, Magnets
HOP publishes a school directory at the beginning of each school year. There is both a printed directory and an on-line app from which to access each family’s information. In the first few weeks of school, you will receive an email with instructions on how to verify your contact information. While the directory is printed in late September/early October, information can be updated at any time via the on-line app. Notepads are convenient pre-printed forms for teacher messages or for the front office if there is a change in your dismissal arrangement. Car magnets are a great way to display your school pride!

Follett Aspen Family Portal
Sudbury Public Schools administers and manages a database system where parents have online access to key components of children’s student records including attendance, schedules, biographical information and competency reports. This system is called the Follett Aspen Family Portal. Parents should contact the front office for your login ID/password. Class pictures, candids, teacher and staff photos, all the big school events, and school portraits of every student will be included in the yearbook to create a beautiful, full-color book of school memories. It’s not just for 5th graders! Students of all ages get excited when they see themselves in the yearbook.

NEW TRAFFIC FLOW
Effective August 31st, the direction of travel into the Haynes side parking lot will change. All cars entering into our parking lot will use the RIGHT LANE at all times. Signs will be placed on school property to remind drivers of this important change. This change was implemented through a collaborative effort between the Sudbury Police Department, the Sudbury School Department and the Haynes School Improvement Council. Our intent is to keep traffic off of Haynes Road to prevent unsafe conditions with the commuting traffic.
DROPOFF AND PICKUP PROCEDURES:
The new Access Lane for Drop Off and Pick Up will be used between the hours of 8:30 – 9:00 AM and 3:15 – 3:45 PM. All traffic will enter and exit Haynes School RIGHT TURN ONLY between the hours of 8:30 – 9:00 AM and 3:15 – 3:45 PM. Please plan your arrival to Haynes School to enter and exit RIGHT TURN ONLY during those hours. NO LEFT TURNS will be allowed during 8:30 – 9:00 AM and 3:15 - 3:45 PM. You will only be able to get in line for Drop Off and Pick Up by entering the Access Lane at the Haynes School Entrance (driving south on Haynes Road from Rt. 117). Once traffic has filled the new front access lane, please line up on Julian’s Way until Drop off/Dismissal begins and traffic starts to flow. Please do not block the school bus entrance during this time. We will have staff outside during both DropOff and PickUp times. Please follow the directions of our staff and designated signs at DropOff and PickUp in order to keep the lines moving. Please help us keep students safe by having your children exit vehicles on the passenger side at all times. Please help keep the line moving by staying in your vehicle, as your children exit or enter the vehicle.
